Print

North Charleston, South Carolina, USA 02 May 2017 -- North Charleston's Kapstone Paper Mill was fined for several reportedly serious workplace safety violations in the two years prior to Sunday's deadly workplace accident.

A review of violation and injury records published by the Occupational Safety and Health Administration (OSHA) shows Kapstone was cited for one violation in 2016, and three in 2015.

The company was fined $1,275 in May 2016 for a "serious" violation related to employees' using appropriate eye and face protection.

In October 2015, OSHA fined Kapstone $1,874 for "serious" violations related to proper breathing protection in dangerous air, authorizations for welding, and labels on hazardous materials.

Lawrence Shiner, 60, of Moncks Corner died in an accident Sunday at the Kapstone Paper Mill. Kapstone officials said Monday Shiner died while doing maintenance work in an offline operating area.

OSHA records for work-related deaths dating back to 2009 show no reported deaths at Kapstone prior to Shiner's death Sunday.

Another worker was injured in Sunday's accident, but released from the hospital.

OSHA requires employers to report all severe work-related injuries, including amputations, hospitalizations, and the loss of an eye.

Kapstone did not report to OSHA any serious injuries or hospitalizations suffered by employees on the job between January 2015 and October 2016.
